  • Flu Shots are recommended every year for everyone ages six months and older. 
    • Influenza Vaccine will be offered at our immunization clinics for children 6 months to 19 years of age.
      
      
• Important Notice: All children are required to have signed parental permission in order to obtain immunizations. If parents are unable to be present at the clinic, the form can be downloaded and signed by the parent or guardian and be taken to the clinic with the child. Children without signed parental permission forms will not be immunized.
